The Committee is developing a model of integrated care for the admitted, home care palliative patients as well as long term care patients. One part of our project involves the development of a palliative care area in the hospital centre.
In the CHSLD's, we have also worked on the procedures of lodging in order to identify a reserved space for the family of the resident in palliative care, to allow them to be present at all times with their loved one.
Various members of the committee have attended conferences and training sessions. In the Fall, we hope to organize in-service training for the health care providers
The Pontiac Community Hospital Foundation Inc, has already received donations for the Palliative Care and the community fundraisers are well under way.
The Rotary Club has accepted to be the primary sponsor. They have created "in memory of" cards for the Palliative Care Project which will be distributed in the local funeral homes.
