Residential and long term care facilities
We have a total of 115 beds for residential and long term care.
Each admission request is evaluated by the CTMSP tool and analysed by a multi local team which determinate the required care hours and recommend an orientation.
A territorial committee orientates and admits the beneficiary in the most appropriate residential resource. The admission at CHSLD will be done within 48 hours as soon as a bed is unoccupied.
The service
“The Manoir Sacré-Cœur”, the Pontiac Reception Centre and the Pontiac Community Hospital (Long term facility) offers lodging, assistance, support and supervision services and other services including nursing, medical, pharmaceutical, psychological, leisure, rehabilitation, respiratory therapy, meals, housekeeping / maintenance, laundry, financial and pastoral services.
Clientele
The services are offered to the adult and elderly population who, by reason of loss of autonomy can no longer live in their homes or foster homes despite the support of families and friends.
Personnel
- Nurse:
- Evaluate the residents health needs
- Provide the appropriate nursing care such as : dressings, injections, blood pressure, vaccination, catheter, antibiotic intravenous therapy, palliative care and other care required

- Coordinate the production, distribution of food, and meals while respecting the standards from public health, safety, hygiene and cleanliness in the work force
- Ensure that all recommended diets are respected as well as resident's food choices and preferences
- Educate residents and their family by providing them with information of healthy eating

Day Centre
- The Day Centre includes :
- Physical, sensorial and cognitive stimulation.
- Thematic information sessions.
- Rehabilitation for maintaining acquired skills.
- Social integration and break isolation.
- Prevention, pairing and socialization.
- These services apply to Elderly persons with autonomy loss.
