ACTIVITIES AND PROJECTS AS OFF NOVEMBER 1ST, 2004

Administration

New By-Law 25
Adoption by the government on the creation of Local Network Services and obtaining an official recognition as a «Réseau local de services »
Chronic under funding of services
Implementation of local public health program for our territory
Implement secure management of our facilities (security for our employees and residents or patients)
Obtain approval for construction of long term care facility in the Shawville area.
Increase in number of patients requiring hemodyalisis. Offer services in the centre

Longterm care services


Implement in our three (3) centres module 1 of the computerize SICHELD system.
Assure the implementation and integration by the staff of a philosophy of "home environment" in our CHSLD’s.
Collaboration with user’s committee to promote quality of life in the CHSLD’s.
Promote empowerment of residents and families in the decision process

Adult program (Young Adult, Adult and mental health)


The absence of a Mental Health Liaison Officer at the hospital
Absence of professional personnel - Hiring of a psychologist
Absence of group and marital therapy
Form a partnership for different avenues of therapy
(ex.: grief, anxiety…).
Integration of adults in the communities Offer support to young adults establishing themselves in the Pontiac.
Absence of specialized transportation and public transportation for client on the «Centre de santé du Pontiac» territory. Creation of service

Family Infant and Youth program


Call the new mother within 24 hours after the birth of her child and do a home visit if it is urgent
Encourage the future parents of Luskville Breckenridge to participate in prenatal courses on the territory
Assure the intensity and the continuity of the "Programme soutien aux jeunes parents"
Offer breastfeeding clinics to promote and to counter difficulties
Begin the implantation of the Baby Friendly Initiative
Offer early stimulation groups to underprivileged children in the different CLSC locations
Support, reinforce and develop family skills with the group session approach
There is an increase of 4% in teen smokers. Promote non smoking habits to teens
Lack of knowledge of the food guide creating bad food habits in kids
Promote healthy eating at home, in schools and also in the community
Promote the Canadian Food Guide.
Few activities for kids outside school time. Organize the activities that were planned by the Local Action Committee for "Quebec en forme"

Home Care, rehabilitation, day centre service


Implementation of a consultation committee for the elderly and persons with autonomy loss
Complete procedure already in progress for home care «basket» of services assessment.
Arrange the formal educational plans in writing.
Find appropriate location for services
Assess potential clients Implementation of a centre in Quyon.
Staff training for heavier clientele.
Implementation of computerized billing tool (SIRTF)

Radiology

Evaluate the feasibility of a numerical filing system for the region.
Allow the reading of reports on the nursing units.

Upcoming inspection by the OIIQ in the Fall of 2004.

Accreditation

Follow up on recommendation of accreditation of 2001.
Prepare for upcoming accreditation process for 2005.

Biomedical

Optimize mechanism
Elaboration of a formal mechanism to establish the priority of the needs.
Elaboration of a formal process for the acquisition of equipment.
Informational technology
To finalize the elaboration and implantation of the policy on security for information technology.
Support the implementation and upgrading of several new information systems.
(SIL-Lab, Sicheld, SIGP,SIGG, IGMF,SIRTF)
Optimize the existing telephone system.
74% of our computer equipment is 6 years old or older.
These systems can no longer be upgraded and can no longer support newer applications. Replace 10% of our equipment annually.

Medical records and admitting
