General Description

 The establishment has merged on October 25, 1996. The services are provided by the following centres:
- a local community service centre (CLSC) and six premises
- two residential and long term care centre (CHSLD) and one long term care centre at the hospital centre (CH).
- a hospital centre (CH)

The merging has allowed to maintain for the Pontiac community, the whole essential services, the home care and the Health recovery.

The regrouping around the four sectors of all the services are based on four objectives:
- encourage an internal coherence of all services intended for a same clientele
- encourage the continuity of services to the population implicated
- encourage modulated approaches in function of the particularities of the territory
- encourage the polyvalency.

Our philosophy

We offer services in both French and English with the same quality warranty. We support the family and encourage the maintenance of the elderly, adults and children and we respect the rights of the clients to be well informed and guided on the services. We ensure confidentiality for the users. We emphasize the development to maintain the skills and expertise of our human resources.

We maintain an organization of services based on integrated and adapted services for the clientele and emphasize our cooperation and implication with community organizations

3 mission :
- The mission of the CLSC is to offer, a primary level of care to the population of the territory. That such services be dispensed in its facilities, in the person’s own environment, in school, at work or at home, they are the external services.
- The mission of the hospital centre is to offer diagnostic services, general and specialized medical care. The services offered in its facilities are the internal services.
- The mission of a residential and long term care centre is to offer, on a temporary or permanent basis, alternative environment, they are internal services.

Considering the socio-sanitary characteristics of the territory, "Le Centre de santé du Pontiac" in the actualization of its mission, shall continue to emphasize:
- the accessibility of health services
- the continuity of health services
- the respect of the rights and freedoms of the person
- the support to the community in the development of resources meeting its needs.
