CLSC Pontiac Foundation ANNUAL PUBLIC INFORMATION MEETING

The CLSC Pontiac Foundation invites the population to its ANNUAL PUBLIC INFORMATION MEETING Wednesday, June 25th 2014, at 11:00 a.m. in the Board Room at the CLSC Mansfield-et-Pontefract, 160 chemin de la Chute, Mansfield-et-Pontefract.

Pontiac Day Center

A specialized resource offering therapeutic activities in order to stimulate and maintain autonomy !
The Day Center is a program offering services to elderly and adult population experiencing a physical, psychological and/or social loss of autonomy. Sessions include preventive and rehabilitative services aiming to maintain their capabilities. Individual and group programs are available as well as adapted transportation and balanced meals.


CLSC Shawville

Services and staff of the CLSC Bryson relocated in CLSC Shawville Since May 7 and 9, 2013, the 50 residents of the Pontiac Reception Center in Shawville, moved to their new living environnment. This move liberates a building owned by the CSSS. As lessee of the CLSC of Bryson, significant extra costs are incurred. For a decade now, it has been planned, in the context of sound financial management, to review this situation in order to bring significant savings to the CSSS of Pontiac. (...)

The Québec Health Record, or QHR, is a tool that allows doctors, pharmacists and some professionals to access essential health information, providing rapid patient management and insuring quality monitoring. The QHR is already operational in the following regions: Capitale-Nationale, Lanaudière, Estrie and Montréal.
